Do You Really Need a SUD of 0 and a VOC of 7 in EMDR?

If you’ve trained in EMDR, you’ve probably heard: “Don’t finish processing until the SUD is 0 and the VOC is 7."


But in real-life sessions, does that always happen? And what does it really mean when clients don’t hit those numbers?

Here’s how I think about SUDs, VOCs, and Phase 4 EMDR processing in a way that’s practical, human, and usable in real sessions.


What is SUD in EMDR?

SUD (Subjective Units of Distress) measures a client’s distress when recalling a memory—from 0 (no distress) to 10 (extreme distress).


  • SUD acts as the gatekeeper: if distress isn’t reduced, nothing else fully integrates.

  • A true SUD 0 indicates the memory is emotionally processed.

  • Sometimes it takes multiple sessions to reach a 0—especially with complex trauma.



Squeezing the Lemon: How to Approach Phase 4 EMDR

During desensitization, we activate the memory to extract adaptive or neutral information.

One metaphor I learned in Basics Training at the Trauma Therapist Institute is “squeezing the lemon”—keep going until no more adaptive info comes out.


Practical Steps:

  1. Track the memory until you see at least two adaptive or neutral responses.

  2. Return to the target and repeat.

  3. Only check SUD after careful work.


Skipping steps often results in incomplete processing. Patience, careful observation, and respect for the EMDR protocol are key.

When a True SUD 0 Isn’t Possible: The Ecological 1

Sometimes, the memory won’t fully reach 0—this is called an ecological 1.


It’s common with:

  • Grief processing

  • Relational or attachment wounds

  • Trauma that activates protective system responses


Ecological 1 isn’t failure—it’s clinical data, telling you:

  • Which parts of the system are resisting

  • Where attachment dynamics or dissociation may be interfering

  • What next steps are needed: revisit, re-conceptualize, or resource stabilization

Understanding VOC in EMDR

VOC (Validity of Cognition) measures how true a positive belief feels to the client.


  • VOC 7 means the adult self is fully present, and the adaptive cognition is integrated.

  • VOC isn’t about blindly reaching a number; it provides insight into:

    • Internal resistance

    • Dissociation or attachment interference

    • Completeness of processing

Body Scan: The Final Integration Check

The body scan confirms nervous system integration:


  • Ask the client to scan their body for tension or unusual sensations.

  • Lingering sensations? Revisit earlier phases.

  • Successful body scans indicate the memory is fully processed.

Why Clinicians Shouldn’t Chase Perfection

Many clinicians feel pressured to hit SUD 0 and VOC 7. Here’s the truth:


  • Numbers are guides, not goals.

  • EMDR is messy, human, and relational.

  • Integration comes from careful observation, patience, and understanding the client system—not just hitting numbers.
